In order to save you time on your first visit to our office, and to assure that you have an opportunity to provide the most complete medical information possible, we have made our basic medical forms available to you online through MyChart. We encourage you to set up your MyChart account and complete the required forms prior to your first appointment. New Patients
As a new patient, our secretarial staff can facilitate scheduling tests, office visits, and other procedures while other staff members serve you by acquiring necessary pre-authorizations, completing employer paperwork, setting up your MyChart account, and filing insurance claims.
During your visits to our office, our nurses and physician assistants often serve as a liaison between you and your physician, allowing him to maximize his time in the operating room and in the office. These highly qualified heath-care professionals will be able to answer many of your questions regarding pre-operative and post-operative care, including recommending treatment, relaying test results, and prescribing medications.
About MyChart – The MyChart online system was designed to enhance and improve your patient health care experience with us. Though not required, MyChart will save you time and effort with every office visit, as well as facilitate mutual communications. For new (or existing) patients without a MyChart account, please call us and we will gladly assist you in getting set up. (Note: email address required)

On your first visit, please bring the following:
- Completed patient information forms (via MyChart or printed – see below).
- X-rays, CT scans, MRI scans or myelograms which you have had done (unless they were done at a Corewell Health, University of Michigan Health West, or Trinity Health Grand Rapids facility).
- Medical records (if they are available to you).
- A list of current medications.
- Your insurance card(s) and a form of identification.
Fees and Insurance
Please present your insurance card at each office visit.
As our office does NOT participate with all insurance companies, please verify your coverage before your appointment.
Participating insurance companies.
We recommend that prior to the initial exam that you verify that your specific group plan has a contract with Great Lakes Neurosurgical Associates PC.
Please be prepared to pay any co-pays at the time of service.
In addition to cash or check, we accept all major credit/debit cards. As a courtesy, we will bill your insurance company for you. Any amounts not covered by your insurance will be your responsibility.
Workman’s compensation or auto insurance
If we are to bill workman’s compensation or auto insurance, we need the following information: insurance company’s name, address, claim number and contact person for that insurance. If workman’s compensation or your health insurance provider determines that you have had an unauthorized office visit and/or surgery, you will be responsible for payment.
